LPCC/LISW

The LPCC/LISW supports the delivery of high-quality behavioral health services through counseling, care coordination, and collaboration. This role includes overseeing behavioral health assessments, providing telehealth support, and implementing behavioral health screening tools to optimize patient care outcomes.



The role is instrumental in fostering interdisciplinary collaboration and ensuring seamless integration of behavioral health into comprehensive care plans. Key responsibilities include collaborating closely with clinical teams to share clinical expertise and program coordination to provide patient-centered, evidence-based care while ensuring compliance with organizational and regulatory standards.
