Job Summary:
Position Title: Medical Assistant, Integrated Care Coordinator

ICC supports daily clinical and operational workflows across multiple service lines including primary care, specialty care, urgent care and health home programs. This position ensures patients receive compassionate, holistic, and well-organized medical care by assisting providers for in house testing while also coordinating support for the patient overall well-being.

Responsibilities:
- Perform patient triage to assess and prioritize medical needs.
- Assist healthcare providers with in-house procedures such as EKGs and PFTs.
- Ensure examination areas are sterile, organized, and properly equipped.
- Sterilize and autoclave medical instruments; uphold infection control protocols.
- Build and maintain positive relationships with patients, caregivers, and care teams.
- Conduct in-person assessments, and surveys to evaluate medical and social needs.
- Develop and implement individualized care
