Position Overview The Patient Outreach & Enrollment Specialist helps patients and community members access healthcare services, health insurance coverage, and available resources.
This role combines patient outreach, healthcare enrollment, appointment coordination, education, and follow-up. The specialist communicates directly with patients and community members, assists with enrollment applications, coordinates appointments, tracks applications and follow-ups, and helps connect individuals with appropriate healthcare services.
The ideal candidate is an excellent communicator who is organized, empathetic, comfortable speaking with patients, and able to manage multiple applications, appointments, and follow-ups while providing a high level of customer service.
Key Responsibilities
Patient Outreach & Engagement
- Conduct outreach to patients and community members to promote healthcare services, health programs, insurance options, and preventive care.
- Educate patients and community members about available healthcare resources and services.
- Conduct follow-up outreach to encourage patients to complete recommended healthcare services and appointments.
- Support initiatives focused on improving patient access, healthcare participation, and care gap closure.
- Build positive relationships with patients, community members, and partner organizations.
Healthcare Enrollment
- Assist patients and community members with healthcare and insurance enrollment applications.
- Help applicants complete required forms and collect necessary documentation.
- Explain enrollment requirements and available options in a clear and easy-to-understand manner.
- Troubleshoot application or enrollment issues and coordinate with the appropriate teams when additional support is needed.
- Follow up on pending applications to ensure the enrollment process is completed.
- Track applicants from initial application through enrollment and access to services.
Appointment Coordination
- Schedule and coordinate patient appointments related to outreach and enrollment initiatives.
- Reschedule appointments when necessary and communicate updates to patients.
- Follow up with patients regarding upcoming appointments and outstanding requirements.
- Help ensure patients are connected with the appropriate medical, dental, behavioral health, or other healthcare services.
Community Outreach
- Participate in community outreach activities, health fairs, networking events, and other healthcare access initiatives.
- Help promote healthcare services, insurance coverage options, and available community resources.
- Build relationships with local organizations and community partners to create referral opportunities.
- Identify community resources and organizations that may help patients access healthcare and support services.
- Distribute educational, enrollment, and outreach materials to patients, community members, and partner organizations.
Documentation & Reporting
- Accurately document patient outreach, enrollment activities, appointments, and follow-up actions.
- Maintain accurate records across databases, tracking systems, and other digital platforms.
- Monitor application and enrollment status to ensure outstanding actions are completed.
- Prepare daily and weekly productivity reports as required.
- Maintain organized records and ensure information is accurate and up to date.
Team Collaboration & Compliance
- Collaborate with internal healthcare, scheduling, outreach, and administrative teams to support patient access and continuity of care.
- Participate in team meetings, planning discussions, training sessions, webinars, and professional development activities.
- Support organizational goals related to patient enrollment, outreach, access to care, and patient engagement.
- Follow established healthcare, privacy, safety, and organizational policies and procedures.
- Maintain confidentiality when handling patient and enrollment information.
- Perform other related duties as assigned.
Requirements
- Able to work in office: Miraflores - Santa cruz, 4 blocks from Ovalo Gutierrez – Non-negotiable
- Able to work full-time: Monday-Friday, 8:00 AM to 5:00 PM PST – Non-negotiable
- At least 1 year of experience in US healthcare environment.
- Experience working with Electronic Health Records (EHR).
- Strong organizational, documentation, and time management skills.
- Ability to work independently while meeting productivity goals.
Preferred Qualifications
- Experience working with Electronic Health Records (EHR), preferably NextGen.
- Experience supporting quality measures, preventive care, or care gap closure initiatives.
