Responsibilities and more: We are hiring a Care Manager to support continuity of care throughout the patient journey, from referral intake through treatment completion. You will serve as a central point of coordination between the organization, patients, attorneys, and providers, ensuring timely communication, accurate documentation, and proper escalation.

Responsibilities

- Manage patient referrals from intake through treatment completion.
- Partner with Scheduling, Front Office,



and Clinical teams to facilitate appointment completion, treatment adherence, and resolution of patient barriers.
- Serve as a liaison with attorneys, referral partners, and external stakeholders, providing case updates and supporting records and billing requests.
- Coordinate internal and external referrals with complete documentation and follow-through.
- Monitor high-risk, high-value, legal, or funding-related cases and escalate appropriately.
- Maintain accurate documentation of patient interactions, referral activity, and escalations.

Requirements
- Experience in care coordination, case management, patient services, or healthcare administration.
- Knowledge of HIPAA, patient confidentiality, and care coordination practices.
- Proficiency with EHRs, case management systems, and Microsoft Office.
- Excellent English communication, sound judgment, and ability to manage multiple cases and deadlines.
- Full-time, remote.
